Updates to the VistaKEANE Resident Care Management and Financial Management systems offer many valuable enhancements that were requested by our clients.

The 4.3.1 update to the Resident Care Management System provides more than 100 enhancements, including additions for the MDS, such as tracking of Significant Change and of completion status. Care Plan was redesigned to make it even more efficient. A Category feature was added to VistaKEANE Progress Notes to extract notes by topic, such as fall, pain, infection, or a RAP.

The Resident Account Management 2.4 update includes a redesigned Collections Note module along with the ability to import Electronic Remittance Advices in the HIPAA 835 format. There were also many enhancements to the RAM Resident Funds application as well.

The latest update to our Financial Management software includes enhancements to Payroll, Accounts Payable, General Ledger & Budgeting, and Materials Management.

The Healthcare Solutions Division (HSD) of Keane, Inc., recently announced that Jeff Jordan was appointed Managing Director of Keane Care. In this role, Jeff will lead operations of the branch and the VistaKEANE product line, focusing on client satisfaction and ensuring that client commitments are met.

With 20+ years of healthcare IT experience, Jeff is an accomplished professional with both hospital and long-term care industry expertise. Jeff's previous experience includes the Keane Hunt Valley office, where he was Managing Director from 1998 to 2001.

In that position Jeff was instrumental in development, support, and marketing of the Resident Account Management module (RAM) and the Leadership Plus product line, the pre-cursor to VistaKEANE.

Since 2001, he has served as Managing Director of the Keane Jacksonville branch, where he helped launch the fully integrated and browser-based Keane InSight hospital information system.

Jeff recently relocated to the Seattle area and will work out of the Keane Care office based in Bellevue WA. He replaces Zoe Bolton who left the company to pursue other opportunities.

Keane has full confidence that Jeff, with his considerable experience and vast industry knowledge, will lead the branch in providing quality and effective solutions that meet the needs of our post-acute care clients.

CMS has released guidance removing federal barriers to using electronic signatures for MDS assessments. Electronic signatures may be used as long as their use is not limited by state or local rules and the facility has policies that authorize them.

Electronic signatures will eliminate the need for handwritten signatures on hard copies of the MDS assessment. Access to clinical records must be available to surveyors.

Ever wonder what happens to all the MDS data submitted by thousands of LTC facilities? One answer is that CMS generates reports showing how frequently specific MDS items, QIs, and RUGs are triggered. Reports were recently added with more current data; they are available in six categories:
- MDS Quality Indicator Report
- Active Resident reports
- Assessment Counts by Reason for Assessment
- RUGs: frequencies and percentages
- RUGs by Reason for Assessment
- MDS Frequencies Report

The Medicare Part A co-insurance rate for 2005 is $114. That rate is effective January 1, 2005, replacing the 2004 rate of $109.50.

The DAVE program is contacting 2000 facilities in 2004. The Data Assessment and Verification (DAVE) program's objective is to assess the accuracy of MDS data. To do so it has been authorized to audit MDS assessments and claims, as well as adjust payment for facilities participating in Medicare and/or Medicaid.

DAVE's preliminary activities showed discrepancies commonly occurred in these MDS sections: G - Physical Functioning, I - Disease Diagnosis, J - Health Conditions, O - Medications, and P - Special Treatments.

New QM: Weight Loss

Starting November 18, 2004, a new Quality Measure is being reported on CMS's Nursing Home Compare Website. It's triggered by a 5 percent or more loss in body weight in one month (a response of 1 at MDS item K3a). Residents receiving hospice care are excluded.

Nursing homes can preview their results before they are posted at www.medicare.gov/NHCompare/Home.asp.

New Surveyor Guidance on Pressure Ulcers

CMS has released a complete replacement for the current Guidance to Surveyors regarding Tag F314 and new language was added to Tag F309 to include certain definitions of non-pressure related ulcers. Click here to read the revision.

Consolidated Billing Refresher

Ten recent CMS Medlearn Matters articles clarify some SNF Consolidated Billing rules. Part of the Medicare Prospective Payment System (PPS), Consolidated Billing (CB) assigns SNFs the responsibility for billing Medicare for virtually all services received by SNF residents in Part A stays. The articles are numbered SE 0431 through SE 0440 and are available at: www.cms.hhs.gov/medlearn/matters/

Beneficiary Notice Delay

The date when use of the new Medicare non-coverage letter is required was extended beyond October 1, 2004. CMS encourages providers to start using the new form, CMS-10055 (SNFABN) now. It is available at this CMS Website: www.cms.hhs.gov/medicare/bni/default.asp.

The Keane LTC Users' Group invites staff from all facilities using the VistaKEANE system to participate in their organization.

The mission of the Keane LTC Users' Group is to provide input to ensure all the products used by Keane Long-Term Care clients meet the ongoing requirements of the post-acute care community. The Users' Group is a self-governing body, independent of Keane.

Membership in the Users' Group is free and individuals determine their level of involvement.
